Background Information about Critical Incidents

What is a critical incident ?

A Critical Incident is any incident or event which has caused or could have caused an adverse outcome for a patient. An example might be a needle stick injury to a patient, a fall off a trolley, the wrong dose of a drug or a patient leaving the department before being seen.

Why bother reporting critical incidents ?

Risk Management is notorious for focussing on patching up events that have already occurred. By developing a database of critical incidents which occur in emergency departments we will be able to focus our attention towards prevention once patterns start to evolve. Furthermore once we have a database of the types of incidents that arise we will be able to focus teaching on areas of need.

How long is this going to take ?

You should be able to complete the report of your incident in just a few minutes. It's dead easy really, just click on the relevant buttons as you go, add some free text in the boxes if you wish and press on the submit button at the end.

What's in it for me ?

We will give you a monthly feedback about critical incidents that you have reported. You have the opportunity by reporting incidents to make a contribution towards changing practice in emergency medicine for the better. This in turn will make life easier for you and better for the patient.
